What to Do After You Receive the FDD

When you receive the FDD, sign, date, initial, and complete all 6 blanks on the Receipt Page (see instructions below) and send Receipt Page to us via FAX: 813-782-1599, or MAIL: 4941 4th Street in Zephyrhills, Florida 33542.

Completing and sending in a Receipt Page is NOT a COMMITMENT to join the chain, to purchase a store, or to anything else at all. Just a FTC required record of us providing you with an FDD.
